Segmental aponeurectomy

What is segmental aponeurectomy?
Segmental aponeurectomy involves making short 1 to 2 cm curved incisions in the skin overlying Dupuytren cord contractures and then removing 1 to 2 cm sections of the underlying cord which then allows the finger to straighten. The wounds are then stitched closed.

When would you do segmental aponeurectomy?
To be honest I rarely do segmental aponeurectomies these days. Needle aponeurotomy is as effective in milder cases, without the need for stitches, and xiapex or open surgery is better for advanced cases. There are occasional cases where I still favour segmental aponeurectomy, for example an isolated painful nodule.
